This position is a part-time (20 hours per week) temporary position supporting Indigenous senior wellness through social prescribing. Social prescribing is a holistic approach where healthcare providers, community-based organizations and groups, friends, and family members can refer individuals to a Community Connector. The Community Connector walks alongside them to build relationships, identify strengths and needs, and connect them to meaningful community supports.
The role requires both remote work and regular on-site presence in community spaces to build rapport, trust, and authentic relationships with Indigenous organizations, Elders, knowledge keepers, and community groups.
The position reports to the Regina Steering Committee on Social Prescribing and provides monthly reports on activities. The position is currently funded until December 31st, 2027.
